Name, phone, email, area of interest, and message are used to review your request and contact you. The form does not ask for a national ID number. If you include health details in the message, they are used only to reply to that request.

Retention and rights

Records are kept for as long as needed to handle the request and any related legal period, then deleted. You may ask to access, correct, or delete your message by phone or email.
